Have you taken a safety training course through TEXO and need a replacement card? Check below for instructions on how to request a new TEXO Safety First, OSHA 10-Hour or 30-Hour, or Medic First Aid/CPR/AED proof of completion card.
TEXO Safety First
To request a new TEXO Safety First card, please email the following information to texo@texoassociation.org:
- First & last name
- Company you worked for at the time you took the class
- Approximate date you took the class
Once your attendance is verified, the new card can be picked up any weekday between 10:00am and 3:00pm at our Dallas location.

TEXO will no longer print lost/stolen badges after 5 years.

OSHA 10-Hour & 30-Hour
To request a new OSHA 10-Hour or 30-Hour card, please email the following information to texo@texoassociation.org:
- First & last name
- Company you worked for at the time you took the class
- Approximate date you took the class
Once your attendance is verified and we have contacted the instructor, we will reach back out to you for payment prior to card reprint. If your instructor is no longer on staff at TEXO, we will provide you with their most up-to-date contact information.
Per OSHA regulations, lost/stolen cards cannot be reprinted after 5 years. You will need to retake the class to renew your credentials – click below to see our upcoming class schedule.

Medic First Aid/CPR/AED
To request a new Medic First Aid/CPR/AED card, please email the following information to texo@texoassociation.org:

- Company you worked for at the time you took the class
